Your Rights

• Be treated with dignity, courtesy, compassion, and respect.
• Receive care without discrimination consistent with applicable law.
• Participate in planning and directing your care to the extent permitted by law.
• Receive understandable information about your condition, services, care plan, expected outcomes, and choices.
• Ask questions and receive understandable answers.
• Know the identity and role of people involved in your care.
• Have privacy and confidentiality respected.
• Receive information about applicable costs, coverage, and payment responsibilities.
• Participate in treatment decisions and refuse care to the extent permitted by law, with information about consequences.
• Receive appropriate pain and symptom management.
• Have cultural, spiritual, personal, and communication preferences respected.
• Involve family, caregivers, or representatives when you choose and when permitted.
• Voice concerns or file complaints without retaliation.
• Access health information and records as permitted by law.

Your Responsibilities

• Provide accurate health and medication information to the best of your ability.
• Tell the care team about changes in condition or medications.
• Ask questions when instructions are unclear.
• Participate in your care plan and communicate goals and preferences.
• Treat staff, patients, and families with courtesy and respect.
• Help maintain a safe environment.
• Provide information needed for insurance and billing when applicable.
