A Seamless, Compassionate Transition
Whether you are transitioning from a hospital stay, a skilled nursing facility, or realizing that your loved one needs more support at home, CHOSEN is committed to making the admission process as smooth and stress-free as possible.
1. The Initial Conversation
It all begins with a simple phone call. When you contact CHOSEN, you will speak with a compassionate care coordinator who will listen to your current situation, answer your immediate questions, and gather necessary information.
What we do: Verify insurance coverage, coordinate with your primary care physician for necessary orders, and schedule a convenient time for an in-home assessment.
2. The Assessment Visit
A Registered Nurse (RN) will visit your home to meet you and your family. This is not a rushed appointment. We take the time to understand your medical needs, emotional state, and living environment.
What we do: Review medical history, perform a physical assessment, evaluate home safety (like fall risks), and discuss your specific goals for care and comfort.
3. Building Your Care Plan
Based on the assessment, our interdisciplinary team collaborates with your physician to create a highly personalized care plan. This dictates exactly what services you will receive and how often.
What we do: Schedule visits from nurses, aides, therapists, social workers, or chaplains as needed. We will clearly explain this schedule to you so you know exactly who is coming and when.
4. Day-to-Day Care & Routine
Your care team begins their scheduled visits. Our focus is on providing excellent clinical care while honoring your personal routines and protecting your dignity within your own home.
What we do: Administer treatments, manage pain and symptoms, assist with daily living activities (bathing/dressing), and provide ongoing education to family caregivers.
5. 24/7 Support & Adjustment
Healthcare needs can change rapidly, often outside of normal business hours. You are never alone. The CHOSEN care team continually monitors your condition and adjusts the care plan as needed.
What we do: Maintain a 24/7 on-call line so you can reach a clinical professional at any time of day or night for emergencies, questions, or immediate support.
